2023-2024 Student Health Requirements:

Students enrolled in the following grade levels for the 2023-2024 school year will require proof of health documentation. Health examinations and vision exams must be submitted prior to October 15th, 2023. Dental forms are due May 15th, 2024, but are accepted at any time prior. Transfer students/students new to the state of Illinois will also require a health and vision exam. Documentation may be uploaded during registration, submitted to your child's building office, or faxed to (815) 648-2339.

First Year Preschool Students- Health physical with immunizations

Kindergarten- Health physical with immunizations, dental exam, vision exam (performed by an optometrist/ophthalmologist)

Second Grade- Dental exam

Sixth Grade- Health physical with immunizations (TDAP & Meningococcal vaccine), dental exam

Ninth Grade- Health physical with immunizations, dental exam

Twelfth Grade- Proof of two doses of the Meningococcal vaccine

Overview

School nursing recognizes the promotion of health and safety, intervention with potential health concerns, case management services, and active collaboration with others to facilitate a student's educational success.

Public health nursing first emerged in the 19th century and pioneered the linkage between home and school. Assignments included acute medical treatment of students in a one-room school house, visiting homes of the children, and providing access to care among community members. Many components of early school nursing (management of contagious diseases and conditions, coordination of health services, promotion of healthy behaviors) are applicable to the health and welfare of students today.
